Virtual Nursing Home
Key Responsibilities
Conduct virtual nursing assessments and follow-up visits with residents and caregivers through secure telehealth platforms.
Monitor patient health status, review vital signs, and coordinate care interventions remotely with onsite staff.
Develop, implement, and evaluate individualized care plans in collaboration with physicians and interdisciplinary teams.
Supervise and guide on-site nursing assistants and caregivers through virtual communication channels to ensure consistent, compassionate care.
Maintain accurate and timely clinical documentation within the electronic medical record (EMR) system.
Provide education and counseling to residents and families regarding medication management, care plans, and health maintenance.
Participate in virtual admission assessments, discharge planning, and interdisciplinary meetings.
Promote compliance with infection control and safety policies through staff training and remote audits.
